In order for refugees to be eligible for a green card/permanent residence they must first complete all required immunizations. At their health screening they will be given immunization records that should look something similar to this:
Immunizations can be scheduled In Guilford County with the Health Department Refugee Health Nurse, H'Lat Mlo, who can be reached at 336-641-4801
Monday-Wednesday: 8:30am - 3:45pm
Thursday: 9:30am - 6:45pm
Friday: 8:30am - 11:45am
Clinic 3 at the Forsyth County Health Department does walk in times for vaccines and can see the whole family.
When all required immunizations have been completed, call H'Lat Mlo to make a green card immunization paperwork appointment, known as form I-693.
At the appointment, each refugee in the family who is 18+ years old must be present with their:
- Social Security Card
- ID
- I-94 form
- $40 per person.
From the appointment, they will leave with a sealed envelope for USCIS (US Citizenship and Immigration Services). Once finished they can schedule an appointment for the green card application. Refer to: http://worldrelieftriad.org/green-card-application
